We accept most health insurance plans, as well as worker’s compensation cases.
Appointments:
Because we are a specialty practice focusing specifically on your Pain Management issues, all patients are required to have a primary care physician. Your primary care physician will be kept informed of your pain management treatment plan and will maintain responsibility for all non-pain related medical issues.
Our satellite sites are gracious to host our clinics; however they are not staffed to answer telephone calls on our behalf. Please direct all calls to our Mercer Avenue Clinic at 309-662-0088.
If you have been given an appointment by our staff, please click on the appropriate provider’s name below and complete our registration packet. Bringing the completed paperwork to your appointment will save your time in the waiting room and keep our staff on schedule. Thank you!
Forms:
Patients who are scheduled for a procedure at Ireland Grove Center for Surgery must complete this packet at least once every six months. Please complete all pages and be sure to sign/date each page that requests your signature. Your Medication list must be completed on their paperwork – do not send a separate medication list. The completed packet must be received by Ireland Grove Center for Surgery at least ONE DAY BEFORE your procedure.
